As you begin to develop an Action Plan, ask yourself the following questions before completing the worksheet.
What is the main purpose or objective of the Action Plan?
What do you seek to accomplish?
Why is it important to your unit or organization?
How does it fit into the strategic plan of the organization?
Who will need to be involved?
Who are your key allies?
What challenges must you address?
What strategies will you use to manage resistance and promote change?
What resources are needed?
Description of the Strategy:
The strategy is to establish a unit based council focused on improving patient care, and promoting the mother-infant dyad through the latest EBP and patient feedback, received via surveys.
What is the issue? To: Make changes to and/or develop strategies for greater impact
TO: The purpose of this intervention is to shift the focus in the postpartum setting from baby friendly to couplet friendly; thus promoting the special bond between the mother-infant dyad, increasing maternal satisfaction/role transition, and supporting infant well-being. Since 1991, the World Health Organization has been pushing to mandate the Baby Friendly Initiative movement throughout hospitals globally (Wieczorek, Schmied, Dorner, & Dür, 2015). While breastfeeding is the uncontested best choice for a newborn, the baby friendly movement pushes breastfeeding as the only choice for the infant, creating tension and anxiety for the mothers when this is unachievable.
What is the action? In a way that : creates greater collaboration and cooperation internally and externally
IN A WAY: Develop a unit based council committed to having monthly meetings where the latest EBP topics regarding postpartum health are reviewed. Where areas of improvement are identified staff will develop intervention trials combining the latest EBP, clinical expertise, and patient’s preferences. Surveys will be sent out both to expecting and recent postpartum moms. These surveys will focus on patient preferences, patient perceived attainment of motherhood, social concerns, and how their hospital stay was perceived, respectively. Surveys will be attempted with the original participants 6 months postpartum to identify rates of maternal satisfaction, maternal frustration, and maternal depression, as well as perceived health of newborn, and perceived bond with newborn, and perceived maternal transition; the council will then analyze and compare the findings between the groups.
What is the Outcome? SO That: institution will meet/make progress toward its goal for…
SO THAT: The goal of these interventions is to focus on the Mother-Infant dyad as a whole, thus increasing patient satisfaction (HCAHP) scores and promoting a smooth transition to, and positive perception of, motherhood during the postpartum period.
