Map IT project
Abstract
Diabetes, obesity and overall health due to lack of activity is becoming an increasing issue over the years. The MAP-IT project empathizes on developing a plan to increase awareness and help the community in the current issues. Even though The CDC has loads of information at the website, the local Department of Health has established a team to reach out the Marion county community called the MAPP. Reaching out to the community by offering free education and at home or group session is the primary goal of the Department of Health of Marion County. It is essential to understand the context of health and illness to the identify the intervention, by exploring the community.( Dreher & Skemp, (2011). The goal is to educate the community by offering free live cooking shows and workouts and healthier food choices.
Mobilize
The community of Marion County needs more available resources, volunteers to educate the community for healthy choice. It is worrisome we are experiencing an increase of new build of fast food restaurants and BBQ places in the county. The goal of the Department of Health of Marion County is to teach the community of how to elect good choice by providing free education at the market place in down town, schools, churches and centers.
Assess
Constantly, when thinking of Florida, we think about elderly retired people. And yes, it is true that we have the greater population migrating from north areas of USA, but Marion County has a diverse population. According to the state of Florida, Marion County is the 17th most populated county consisting of: 19.4% of the population under the age of 18, and 25.5% of the population over the age of 65. This is compering of Florida State of 21.3% under the age of 18 and 17.3% over 65 years respectably (Florida Legislature Office of Economic and Demographic Research (2016).
The Department of Health has created a project called MAPP which is community-wide strategic planning process for improving community health and local public health systems. The goal of the program is organize the process, partnership development and visioning to improve the community. It is composed of four critical assessments follow: community health status assessment, forces of change assessment, local public health system assessment it is an effort achieve the National Public Health Performance Standards Program (NPHPSP), and assessment of community theme sand strengths (Florida Department of Health, 2012).
Unfortunately, the community requires more resources than what the Department of Health is offering at the moment. The MAPP program consists of two full time employees that would develop a plan and coordinate activities. The teaching and reach out to the community only takes place once a week and it is only for two to four hours at the time. There are volunteers available, but the DOH needs to employee more personal to obtain a better outcome from the project.
The problem the community has is: diet, low physical activities, portion control. All these concerns can be address if they are more coaching session provided not only from the DOH, but also by other entities in the county, and more volunteer and personal is available.
Planning and Implementation
Planning an improvement of a community is not an easy task especially when the resources and personal are not sufficient. This will be the challenge of the MAP IT project. Since there is an establish program available in the county to improve the community, the missing ingredient to improve the vision is personal. The initiative will first, obtain more volunteer each week. The volunteers will consist of nursing professionals that would participate closely with each community participant. There would be an increase of sessions or activities provided by the Department of health depending on how many professional volunteer are available. Each nursing professional would acquire an individual or family to coach. Close contact with between Heath professional and individual with be monitored during six months. At the end of the period, the individual or Family would be assess on overall improvement of health.
Tracking
After the period of six months, the family or community individual will be assessed by the MAPP coordinator to investigate the progress or education acquired. The following information will be gathering.
